What can you expect?
Step 1: Initial Assessment
Following your physician’s orders, our team will send a Registered Nurse within 48 hours to conduct an initial assessment. This assessment will help us evaluate your needs and may involve reviewing medical history, medications, and discussing your goals and preferences.
Step 2: Care Planning
Upon admission, we will curate a specific care plan tailored to your unique needs and your physician’s preferences.
Step 3: Carry Out The Plan
Once the care plan is in place, a Rose Haven clinician will provide the necessary services in the comfort of your own home. This may include skilled nursing, rehabilitation therapies, medication management, wound care, assistance with activities of daily living, and more.
Step 4: Monitoring and Evaluation
We pride ourselves in the importance of effective communication and collaboration with your healthcare team. Our clinicians will work closely with your primary care physicians and specialists to ensure seamless continuity of care. We will regularly keep them informed about your progress, any changes in your condition, and adjust care plans as needed.
Visits will occur less frequently as you continue to improve.
